Mafia: The Old Country plunges players into early 1900s Sicily, exploring the origins of organized crime.

Our hero, Enzo Favara, rises from a brutal childhood of forced labor in sulfur mines, seeking a better life. He finds his chance by joining the formidable Torrisi crime family, swearing an oath of loyalty.

The primary antagonists are rival crime families, notably the Spadaro family, and their ruthless leaders who wage violent turf wars. Enzo's ultimate goal is to prove his worth, become a "made man" within the Torrisi family, and carve out a life of power and respect, whatever the cost.

This gritty narrative promises a blend of close-quarters knife combat, stealth, and period-authentic gunfights, immersing players in a treacherous historical underworld.
